There are loads of possibilities, but I guess many of the are so well known that it would be pointless describing them again here, e.g. Bhopal or Chernobyl.

Tay Bridge Disaster, perhaps? Though you could make a whole list of rail disasters alone.

Emily-Yes, Johnstown totally belongs here. That flood happened due to the negligence of the rich cats from Pittsburgh who purchased an abandoned reservoir near Southfork and converted it into a private resort lake. Some of the modifications they made to the dam at the reservoir included lowering the dam to make its top wide enough to hold a road, and putting a fish screen in the spillway (that also trapped debris). These alterations increased the vulnerability of the dam. Even when told the dam might not hold unless they fixed it, they didn't fix it. Big rain. Dam broke. Flood killed 2,200.

Centralia is a bit of a gray area because there isn't enough evidence for either theory over why the mine fire started.

Does Johnstown really fit the theme of this article? Disaster yes, but not of complete negligence like the others mentioned here. I think Centralia (<a href="http://en.wikipedia.org/wiki/Centralia,_Pennsylvania)" target="_blank">http://en.wikipedia.org/wiki/Centralia,_Pennsylva...</a> fits the theme, the still burning mine fire there could have been stopped or avoided long ago... instead Centralia is pretty much a ghost town now.
